Thieme-Verlagsdatenbank contains bibliographic data and abstracts (approx. 63 %) belonging to documents from approx. 180 medical and scientific journals published Thieme-Verlag since 1981. Database documents are all linked with the corresponding original full texts as PDF, available for a fee. A further specialty is the link of the full texts plus the corresponding publication information from other bibliographic databases like MEDLINE or EMBASE. DIMDI provides the Thieme-Verlagsdatenbank in cooperation with the German National Library of Medicine (ZB MED).

AB: HISTORY AND FINDINGS: A 37-year-old man with a 19-years history of injection drug use (IDU) who had acquired a chronic hepatitis C virus (HCV-) infection 9 years ago, entered the German clinical study on heroine assisted treatment ("Modellprojekt zur heroingestützten Behandlung Opiatabhängiger"). Before study onset he received buprenorphine maintainance treatment, while at the same time engaging in illicit IDU (heroine, cocaine). He lived in a caravan and was on social welfare. INVESTIGATIONS: PCR revealed a genotype 2 and an HCV-viral load of 310 000 IU/ml. Liver biopsy showed a moderate chronic active hepatitis and a mild portal fibrosis without signs of liver cirrhosis. DIAGNOSIS, TREATMENT AND COURSE: Within the heroine-assisted treatment program the patient injected heroine under medical supervision several times a day and attended the standardized psychosocial program that comprised an intensive education on HCV-infection. Within a period of ten months of physical and social stabilization he managed to stop illicit drug use, found stable housing and started to work. We then initiated treatment of HCV-infection. Subcutaneous pegylated interferon alpha-2a, peroral ribavirin and intravenous heroine were administered as directly observed therapy. Based on the close mashed care of the heroine assisted treatment setting, side effects were well controllable and reversible after the end of antiviral therapy. A sustained response was obtained. CONCLUSION: After careful indication, heroine-assisted treatment with particularly intensive medical and psychological care can offer appropriate conditions for a save and successful treatment of hepatitis C as well as for a sustained result.
ANAMNESE UND KLINISCHER BEFUND: Ein 37-jähriger, seit 19 Jahren intravenös drogenabhängiger Patient mit seit 9 Jahren bekannter chronischer Hepatitis-C-Virus- (HCV-) Infektion wurde in das Modellprojekt zur heroingestützten Behandlung Opiatabhängiger aufgenommen. Er befand sich in einer Substitutionsbehandlung mit Buprenorphin, konsumierte regelmäßig Heroin und Kokain, lebte in einem Wohnwagen und erhielt Sozialhilfe. UNTERSUCHUNGEN: Die PCR erbrachte eine HCV-Viruslast von 310 000 IU/ml sowie einen Genotyp 2. Die Leberbiopsie zeigte eine mäßige, chronisch aktive Hepatitis mit leichter portaler Fibrose ohne zirrhotischen Umbau. DIAGNOSE, THERAPIE UND VERLAUF: Im Rahmen der heroingestützten Behandlung injizierte der Patient mehrfach am Tag unter ärztlicher Aufsicht Heroin und nahm die standardisierte psychosoziale Begleitung wahr, die eine intensive Schulung bezüglich der HCV-Infektion beinhaltete. Nach einer 10monatigen Phase gesundheitlicher und sozialer Stabilisierung, in der es ihm gelang, den Beikonsum einzustellen und eine Wohnung sowie Arbeit zu finden, wurde die antivirale Behandlung der chronischen HCV-Infektion eingeleitet. Pegyliertes Interferon alpha-2a wurde subkutan injiziert, Ribavirin peroral verabreicht. Die Medikamenteneinnahme erfolgte zusammen mit den intravenösen Heroininjektionen unter Sicht. Die Nebenwirkungen der antiviralen Medikamente waren im Rahmen der heroingestützten Behandlung gut beherrschbar und nach Therapieende reversibel. Es gelang eine anhaltende Viruselimination. FOLGERUNG: Eine heroingestützte Behandlung kann durch intensive Betreuung und häufige Arztkontakte nach sorgfältiger Indikationsstellung eine gute Voraussetzung für eine sichere, erfolgreiche und nachhaltig wirksame Behandlung der chronischen Hepatitis C bieten.
