EMBASE (EM10, EM05, EM00, EM95, EM90, EM83, EM74, EM47)

EMBASE (Excerpta Medica Database) covers worldwide literature, especially from Europe, in the field of human medicine and related fields with special emphasis given to drug research, pharmacology, pharmacy, pharmacoeconomics, toxicology, basic biological research, health policy and management, public, occupational and environmental health, and substance dependence and abuse. Since February 2010, we offer EMBASE with its ClassicFiles. This archive completed EMBASE to 1947. The database contains all citations of the printed Excerpta Medica sections as well as additional literature. Sources include approx. 7,000 international journals from 70 countries (including journals of MEDLINE). Bibliographic information, indexing terms, and abstracts (approx. 80 %) are searchable.

The database contains limited coverage of normal psychology (unless related to pharmacology), veterinary and dental medicine.

Since middle of 2010 EMBASE offers all Journals that are indexed by MEDLINE. The manufacturer Elsevier expands EMBASE with 2,000 journals to include residual journals. The new entries come from the MEDLINE database of the U.S. National Library of Medicine (NLM). You can recognise the new documents by the entry "MEDLINE" in the Subunit (SU) data field. In the DIMDI ClassicSearch, you can search for it by using the command FIND SU=MEDLINE. In the DIMDI SmartSearch, you must use the advanced search for this purpose and enter SU=MEDLINE in the input line.

In MEDLINE und EMBASE parts of the journals were already redundant before. These Documents are not labelled by the entry "MEDLINE" in the Subunit SU.

The subheadings formerly links (qualifiers) are divided into two groups: drug subheadings and disease subheadings. Drug subheadings are assigned to the drugs in category D (chemicals and drugs) (so-called EMTREE drug terms) and disease subheadings are assigned to illnesses (so-called EMTREE disease terms). There are 14 disease subheadings and 64 drug subheadings, including 47 links for the "routes of drug administration", which were introduced in 2000. The "drug therapy" subheading can be assigned to both drugs and illnesses during the search. The subheadings/links were introduced in 1988, but can also be searched for earlier years however, although the accuracy is not so high in this case. Subheadings terms can also be indexed as normal EMTREE terms and are therefore directly searchable (e.g.: FIND CT=buccal drug administration). (List of subheadings/qualifiers: see appendix).

Please note for documents of EMBASE Classic (publication year 1947 to 1973):
No subheadings (qualifiers) attached to CT.
Data fields BTI, CDEV, DMN, PII, PU, ANR, CF, EISSN, ISBN, NCT and URL are not available. The fields were added at a various later dates. Data fields EC, ET, DN, MN, CO, CY, CYJT, EDR, RN und SD are available from 1970.

Standing orders (SDI) are possible for Premium customers.

AB: Multiple clinical trials have demonstrated beta -blockers improving survival after myocardial infarction (MI). Patients with "bradycardia-related" contraindications to beta -blockers, such as those with asymptomatic bradycardia or AV conduction abnormalities, have been excluded from clinical trials of beta -blockers and continue to be excluded from post-MI beta -blocker therapy in routine clinical practice. These patients tend to be elderly and have a high 1-year mortality. If beta -blockers provide benefit to the post-MI patient independent of their heart rate-lowering effect, then these patients could benefit substantially from initiation of beta -blocker therapy. However, in this particular group of patients, beta -blockers can be safely initiated only if more severe or significant bradycardia can be prevented by pacemaker implantation. It is unclear whether adverse effects related to pacemaker implantation could also negate some or all of the hypothesized benefit of beta -blocker therapy. Although beta -blockers are particularly effective in the elderly, the benefit of beta -blocker therapy in patients with bradycardia-related contraindications to beta -blockers has not been established. The PACE-MI trial is a randomized controlled trial that will address whether beta -blocker therapy enabled by pacemaker implantation is superior to no beta -blocker and no pacemaker therapy after MI in patients with rhythm contraindications to beta -blockers or in those who have developed symptomatic bradycardia due to beta -blockers. The trial will randomize 1124 patients to standard therapy (not to include beta -blockers as patients must have a contraindication to be enrolled) or standard therapy plus pacemaker implantation and beta -blocker. The primary end point is the composite end point of total mortality plus nonfatal reinfarction. © 2008.
