| Die gerinnungshemmende Wirkung von Warfarin bzw. Acenocoumarol kann bei gleichzeitiger Behandlung mit Cimetidin innerhalb einiger Tage zunehmen. Blutungskomplikationen können auftreten. | ||
| ACTN WIRKUNGSMECHANISMUS : | ||
| Cimetidin hemmt die Cytochrom-P-450-abhängigen Enzyme, die den oxidativen Metabolismus von Warfarin uns Acenocoumarol katalysieren. Die übrigen H2-Blocker hemmen diese Enzyme nicht in klinisch relevantem Ausmaß. Das in Europa weit überwiegend verwendete Phenprocoumon, das hauptsächlich als Glucuronsäure-Konjugat ausgeschieden wird, ist von dieser Wechselwirkung nicht betroffen. | ||
| MAS MASSNAHMEN : | ||
| Während der ersten Tage einer gleichzeitigen Behandlung mit Warfarin oder Acenocoumarol und Cimetidin sollen die Blutgerinnungsparameter besonders sorgfältig überwacht und die Dosis des Antikoagulans nach Bedarf angepasst werden. Alternativ können Phenprocoumon oder andere H2-Blocker (Famotidin, Nizatidin, Roxatidin) gegeben werden. | ||
| KOM KOMMENTAR : | ||
| In Einzelfällen sind unter gleichzeitiger Behandlung mit Ranitidin und Warfarin Blutungskomplikationen aufgetreten. Ob hierfür eine Interaktion mit Ranitidin oder andere Gründe verantwortlich waren, ist aber nicht geklärt. | ||
